Community Nursing Care

Our qualified nursing team provides disability-related nursing and clinical supports in the participant’s home or community. Supports are tailored to individual health needs, care plans and goals, with a focus on safety, comfort, independence and quality of life.

Our services may include:

  • Medication management
    – Oral medication support
  • Subcutaneous and intramuscular injections
  • Wound care and wound management
  • Dysphagia-related clinical support
  • Pain management support
  • Palliative nursing support, where appropriate

High Intensity Daily Personal Activities

Our trained and competent support team provides high intensity disability supports for participants with complex support needs. Supports are delivered in accordance with the participant’s current care plan, clinical instructions, assessed needs and individual preferences.

Our supports may include:

  • Complex bowel care
  • Enteral feeding support
  • Severe dysphagia management
  • Urinary catheter management
  • Subcutaneous injections
  • Complex wound management
  • Tracheostomy support
  • Ventilator-related support

High intensity supports are provided only where they are within our service scope and workers have completed the required participant-specific training and competency assessment.

Supported Independent Living (SIL)

Our Supported Independent Living services provide individualised daily living support to help participants live safely, confidently and as independently as possible in their own or shared home.

Our supports may include:

  • Personal care and daily routines
  • Meal preparation
  • Household activities
  • Medication-related support as authorised
  • Community access
  • Support with appointments and activities
  • Development of daily living skills
  • Support with choice, independence and decision-making

Implementing Behaviour Support Plans

Unity Care Support works collaboratively with participants, Behaviour Support Practitioners, families and support teams to implement current Behaviour Support Plans consistently and safely.

Our supports may include:

  • Participant-specific worker training
  • Positive behaviour support strategies
  • Behaviour monitoring and data collection
  • Incident documentation and review
  • Supporting skill development and
  • meaningful participation
  • Implementation and monitoring of authorised restrictive practices where applicable

Personal Care & Daily Living

Our team provides respectful and person-centred assistance with everyday personal activities, supporting participants to maintain dignity, independence and choice in their daily routines.

Our supports may include:

  • Showering, dressing and grooming
  • Personal hygiene
  • Mobility and transfers
  • Toileting and continence-related assistance
  • Daily routines
  • Meal-related assistance
  • Medication prompting or assistance as authorised
  • Support to maintain independence in everyday activities

Support Coordination

Our Support Coordination services help participants understand and implement their NDIS plans, connect with appropriate services and build confidence in managing their supports.

Our supports may include:

  • Understanding and implementing the NDIS plan
  • Connecting with suitable providers and community services
  • Coordinating multiple supports
  • Addressing service delivery issues
  • Building capacity to manage supports independently
  • Supporting preparation for plan reviews and changes in support needs

We work alongside participants to strengthen choice, control and confidence in navigating their supports.

Physiotherapy

Our physiotherapy services support participants to improve and maintain mobility, physical function and independence based on individual needs and goals.

  • Mobility and movement assessment
  • Strength and balance
  • Falls prevention
  • Functional assessment
  • Individualised exercise programs
  • Mobility and positioning advice
  • Assistive equipment recommendations where appropriate

We aim to support participants to maintain a clean, safe and comfortable home environment.

House & Yard Maintenance

Our house and yard maintenance supports assist participants with essential outdoor and household maintenance activities they are unable to safely complete themselves.

Our supports may include:

  • Lawn mowing
  • Whipper-snipping
  • Basic garden maintenance
  • Trimming and yard tidying
  • Nature strip maintenance
  • Removal of garden waste where agreed

Services are provided according to the participant’s assessed needs and agreed support arrangements.
